In an effort to increase health awareness and make it a part of our daily lives, Demco is introducing a new points-based incentive program that will begin on May 1, 2008.

To be eligible for the S.M.A.R.T. HEALTH program you must be a Demco employee and you must participate in the Demco Health Fair on May 1 & May 2.

Participants are also required to:

  1. Have an INITIAL ASSESSMENT. This includes:

  •  
    • Lab screening at the Health Fair to measure cholesterol, LDL, HDL, triglycerides and HDL ratio and glucose.

    • If you have recently had blood drawn at your doctor’s office, you will not be required to screen again at the Health Fair. You do, however, need to bring in your lab results to the Health Fair, so it can be included with your Personal Wellness Profile

    • Demco will cover 100% of the cost of this screening for all FULL-TIME employees (60 day introductory period must be completed). Cost for all other employees will be $25.00.

  •  
    • Height, weight and body mass composition

    • Blood Pressure and pulse

    • This assessment will be repeated in 12 months for follow-up. Complete a Personal Wellness Profile. This needs to be filled out and returned prior to the Demco Health Fair. Each employee who completes this profile will be rewarded with $10.00 in cash!!!

Obtain a certain amount of points each week in the following categories:

  1.  
    1.  
      • PHYSICAL EXERCISE: this includes many activities such as walking, running, biking, swimming, weightlifting, etc.
      • BONUS POINTS: the activities in this section are things that we are either required to do or should be doing to optimize our own health and well-being, such as regular physicals, cancer screens, stop smoking, etc.
      • NUTRITION: this category includes items that encourage healthy eating.
  • Turn in calendars each month:
  • Monthly calendars, with weekly points, must be turned in by the 10th of each month. NOTE: If you miss turning in your calendar by the 10th, you should still turn in it for credit toward the years total.

  • A drawing will be held each month for all participants who turn in their calendar on time, with the required number of weekly points. The winner will receive a $50.00 GIFT CARD. You cannot win this drawing more than once per year.

  • Participants that meet the monthly requirement for the entire year will be entered into a drawing for an additional $100.00 GIFT CARD.


You must get at least a minimum of:

  • 8 PHYSICAL EXERCISE points each week and

  • 16 NUTRITION points each week

  • You can use 2 BONUS POINTS per week towards either the Physical Activity or Nutrition point requirements

  • You are required to total your own points – this is your responsibility!

PARTICIPANTS ARE ALSO ENCOURAGED TO SET YOUR OWN PERSONAL GOALS FOR WEIGHT LOSS, BODY FAT REDUCTION, REDUCED BLOOD PRESSURE, ETC.!

Our goal is for you to make wellness activities a part of your daily routine. But should illness or injury strike that doesn’t mean you’re out for the year. Let us know your situation. If it is a temporary situation you may be granted grace, until you can resume your activities again.

YEAR END REWARDS:

The S.M.A.R.T. HEALTH program will continue for 1 year. In May 2009, each participant who has met the minimum requirements for at least 6 months during the year and accumulated the following points will be awarded additional prizes as follows:

1250 points 2 night hotel package
750 points 1 night hotel package
500 points $50 VISA gift card
250 points $25 VISA gift card

This program does require some commitment, self-motivation and record keeping on your part, but it should help you make “smarter” choices about your health. It’s a WIN-WIN deal! Why not give it a try?
